Anonymous Questionnaires
Anonymous questionnaires are answered through a public link, without an account, an enrollment or a paired device. They suit screening forms, satisfaction surveys and public health questionnaires where no personal data should be collected.
Requirements
- The questionnaire's identity policy is Anonymous (chosen when the questionnaire is created; see Designing Questionnaires).
- The questionnaire is published.
- Anonymous questionnaires are enabled on your instance by the administrator.
Generating a link
- Open the questionnaire and go to Publishing & Enrollments → Anonymous Links.
- Under Generate New Link choose the Language the page opens in and, optionally, a Custom Title for the browser tab.
- Optionally set a Start Date (before it the page says the questionnaire is not yet available) and an End Date (after it the page no longer accepts answers).
- Select Generate Link.
The link appears in Generated Links with its language, creation date and availability window. From the list you can Copy URL, Show QR Code (for posters or printouts), Open Link to test it, or Delete it. Deleting a link stops it working for everyone who has it; links past their end date are marked Expired.
Each link is a self-contained page: it embeds the questionnaire engine and the questionnaire content, so it keeps working exactly as generated even if the questionnaire is later edited into a new version.
Consent gate
If the questionnaire has legal documents configured (Configuration → Legal), the page shows them before the first question. Respondents must accept every document marked mandatory to continue; optional documents can be dismissed. The generate form reminds you how many documents will be shown and how many are required.
What respondents see
- The consent gate, if any.
- The questionnaire in the selected language, rendered by the same engine as in the mobile app, with the ability to switch to another questionnaire language.
- A completion message after submitting, or an error message with a retry option if the submission could not be sent.
No sign-in, no cookies with personal data and no device pairing are involved.
How responses appear
Anonymous submissions show up in the questionnaire's Results with no patient. Their Source is recorded so they can be filtered separately from web app and mobile app submissions. They take part in charts, datasets and analytics like any other submission, and can trigger Workflows and Webhooks.
Privacy considerations
- The engine does not collect names, contact details or device identifiers. Do not ask for them in the questions unless your policy allows it.
- The availability window is enforced on the server as well as in the page, so an expired link cannot be used to submit.
- A link can be answered any number of times; if you need one answer per person, use an identified or pseudonymised questionnaire with enrollments instead.
Troubleshooting
| Symptom | Likely cause | What to do |
|---|---|---|
| "Not an anonymous questionnaire" | The identity policy is Identified or Pseudo-Anonymised | Create an anonymous copy (Duplicate and change the type) or a new anonymous questionnaire. |
| "Anonymous questionnaires are not configured" | The feature is disabled on the instance | Ask your administrator to enable it. |
| Link opens but says "not yet available" | The start date is in the future | Wait, or generate a new link without a start date. |
| Submissions fail with an error overlay | The link has expired or the questionnaire was archived | Generate a new link on a published questionnaire. |
| The page shows an old wording after you edited the questionnaire | Links embed the version they were generated from | Generate a new link from the new published version and retire the old one. |
