Designing Questionnaires

The designer is where you build the content of a questionnaire: its sections, questions, texts, translations and the settings of each question. Logic and scoring have their own page: Logic, Variables & Scoring.

Creating a questionnaire

From a folder, choose Create New Questionnaire and fill in the wizard:

SectionFields
Basic InformationQuestionnaire Name (internal, unique) and an optional description.
Questionnaire TypeThe identity policy: Identified, Pseudo-Anonymised or Anonymous (see below).
Language SettingsQuestionnaire Default Language, Use Multiple Languages and the list of Questionnaire Languages. Options follow the folder configuration.
Questionnaire EngineWhich engine renders the questionnaire and its engine-specific configuration. Only engines allowed in the folder are offered.
Destination FolderWhere the questionnaire is stored.

Other ways to start:

  • Import Questionnaire from a file previously exported with Export as File or Export JSON.
  • Duplicate an existing questionnaire from its menu to copy its content into a new draft.
  • Ask the AI Assistant to draft a questionnaire from a description.

Identity policy

PolicyWho can answerHow answers are linked
IdentifiedEnrolled patients or users who sign inAnswers are attached to the responder's record.
Pseudo-AnonymisedEnrolled responders who sign inThe responder's identity is permanently detached from the answers.
AnonymousAnyone with a public linkNo personal data is collected; see Anonymous Questionnaires.

The questionnaire workspace

Opening a questionnaire shows its name, status badge (Draft or Published) and a navigation with: Designer, Scheduling, Publishing & Enrollments, Results, Versions & Relations, Simulation, Functions, Variables and Configuration.

While the questionnaire is a draft you can edit freely. Once published it is locked; choose Enable Editing to continue in a new version (see Versions & Publishing).

Structure: sections and questions

A questionnaire is a list of sections, each holding an ordered list of questions. Sections matter beyond layout: schedules can include only selected sections, so a long questionnaire can be split into parts that patients answer at different times (see Scheduling & Enrollments).

Section actions: Create New Section, Edit Section Title, Move Section Up/Down, Collapse/Expand, Lock/Unlock Section (prevents accidental edits) and Delete Section.

Question actions: Add Component… (opens the question type picker), Copy Question, Paste Question from Clipboard, Move Question Up/Down and Delete Question. Questions can also be dragged to reorder.

The left hierarchy panel offers three views: Tree (structure), Description (a readable outline) and Flow Map (a diagram of navigation logic). The toolbar can filter questions by type and switch between the editing canvas, a list view and an HTML preview.

Question types

The set of question types depends on the engine selected for the questionnaire. The default engine provides:

TypeTypical use
TextFree text, short or long.
NumericNumbers with optional minimum, maximum and units.
Single ChoicePick one option from a list.
Multiple ChoicePick several options.
SliderA value on a scale with labelled ends.
DateA calendar date.

Custom engines can add further types; see Custom Question Components.

Question settings

Select a question to open the side panel with three tabs:

  • Details: Reference Name (the identifier used in logic and datasets; must be unique), Question ID, Component Type, internal Description (never shown to respondents) and Is optional to answer. Below it, Component Specific Configuration exposes the settings of the chosen type: option lists, ranges, placeholder text, validation messages, formatting.
  • Actions: the logic that runs when a question is entered or answered.
  • Preview: how this single question renders.

Question text and option labels are rich text and can include emphasis, lists and links.

Working with languages

With multi-language mode on, the toolbar language selector switches the language you are editing. Content typed in the default language is the reference; other languages are translations of it.

  • Translated View is a language-specific editing layer: it lets you adapt wording per language without changing the structure.
  • Per-Language Override in the question configuration lets you change component settings (for example option labels or validation messages) for one language only. Add missing configuration from default language copies what is not yet translated.
  • Split View shows the default language and the current translation side by side.
  • Removing a language from Configuration → Languages permanently deletes its translations.

Respondents can switch between the questionnaire's languages while answering; their preferred language is used first.

Preview

The Preview panel renders the questionnaire with the real engine. Options:

OptionEffect
Device / Phone FrameSimulate phone sizes and show a device frame.
Show Current Section OnlyRender just the section you are working on.
Show Current Question OnlyRender just the selected question.
Auto Refresh / RefreshRe-render on every change, or on demand.

Preview is visual only; to test logic with answers use Simulation.

Saving, undo and shortcuts

Changes are held in the browser until you select Save Questionnaire; a banner reminds you of unsaved changes. Undo and redo cover structural and text edits.

ActionShortcut (macOS / Windows)
UndoCmd+Z / Ctrl+Z
RedoCmd+Shift+Z / Ctrl+Shift+Z
Delete selected question or sectionCmd+Backspace / Ctrl+Backspace

Validation runs continuously. Duplicate reference names, missing texts in a language or invalid component configuration appear in the validation panel and must be fixed before publishing.

Questionnaire configuration

Configuration groups settings that are not part of the content:

TabContents
GeneralName, description, parent folder, questionnaire type, and lifecycle actions (Export as File, Archive, Restore, Delete Permanently).
LanguagesDefault language and translation languages for this questionnaire.
Questionnaire EngineThe engine in use, its capabilities and supported question types, and Change Questionnaire Engine.
LegalTerms and Conditions, Privacy Policy and Informed Consent texts per language, each optional or mandatory (see Consent Documents).
Manage AccessCollaborators and their access level for this questionnaire.
